Obtaining an Agency

On submitting your first piece of business, please complete a PTFS Business Submission Form for PatientChoice (a generic copy of which is at the foot of this document). You will then be allocated a sub-agency number, which will enable you to track the progress of your applications and commission payments. Please clearly state your sub agency number on all future applications.

Please note that Intermediaries who are Directly Regulated with FSA may need to complete a separate agency application with provider to benefit from any PTFS special terms. This does not apply to Appointed Representatives of PTFS.
